Sign Note and Request Co-Signature

The co-sign feature allows a provider to sign a note and forward it to another provider for co-signature. Co-signing is a helpful documentation tool for practices with students/interns or when multiple providers see the same patient during the same encounter.

Access Clinical Note

Open notes can be accessed from various locations: the patient's appointment on the Dashboard, Open Notes, or the patient's Notes tab.

  1. Hover over the Platform icon and click on Dashboard. The Dashboard opens.
  2. Click the In Office or Finished tab. The patient appointments displays.
  3. Click on the patient's appointment to view the appointment card.
  4. Click View Note. The saved note opens.

Sign Note and Request Co-Signature

  1. Review the clinical note and document or modify the note, as applicable.
    icon_tip.png Tip: Once the clinical note is signed, patients can access their health record in the Patient Portal to review their summary of care. If applicable, click to select "Mark as Confidential Note" to exclude the note from the Patient Portal.
  2. Click the Sign up arrow and select Sign & Request Co-sign. The Request Co-sign pop-up window opens.
  3. Click the drop-down arrow and select the co-signing provider.
  4. Click Sign & Notify. The note displays with a status of Needs Cosign in the patient's notes and Open Notes for both the initial and co-signing provider.​
