Archive or Restore Custom Form

System administrators can archive custom forms that are no longer needed. The forms will be marked as archived and will not be available to send to patients. If necessary, restore an archived form.
Available for Engage subscribers and accounts with access to Patient Intake.
Deactivate Custom Form
- Hover over the Usericon, and click on Practice Settings. The Practice Settings page opens.
- Click Patient Intake. The Patient Intake settings page opens.
- Click the more option icon and select Archive form. A green confirmation message displays.

Restore Custom Form
- Hover over the User icon, and click on Practice Settings. The Practice Settings page opens.
- Click Patient Intake. The Patient Intake settings page opens.
- Click the more option icon and select Restore from. A green confirmation message displays.
