Diagnosis Codes

Diagnosis codes are used within the healthcare industry to identify specific medical diagnoses of patients receiving treatment. These codes correspond with the code list published quarterly by the Centers for Medicaid and Medicare Services branch of the U.S. Department of Health and Human Services (HHS) and are updated in Tebra per guidelines. However, you can deactivate or edit the local settings of these diagnosis codes as needed. You can also create new diagnosis codes for any unlisted codes.

icon_warning.png Important Note: Diagnosis codes are not practice specific and are shared across all practices within the Tebra account. Any additions or modifications will affect the entire Tebra account and all practices associated with it.

You always begin with finding the diagnosis code before creating, editing, deactivating or deleting a diagnosis code.

Find Diagnosis Code

  1. On the top menu bar, click Settings > Codes > Find Diagnoses. The Find Diagnosis window opens. By default, ICD-10 code list displays. If ICD-9 codes are needed, click the drop-down arrow and select ICD-9.
  2. Enter all or part of a keyword of the diagnosis in the Search box then click Find Now. The search results are listed.

New Diagnosis Code

  1. To prevent duplication, verify the diagnosis is not in the system then click New at the bottom. The New Diagnosis window opens.
    Note: Confirm the correct code list (e.g., ICD-9, ICD-10) is selected.
  2. Enter the information.
    • Code: The code for the diagnosis.
    • Official Name: The billable name for the diagnosis.
    • Local Name: A user-friendly name for the diagnosis that appears on encounters.
    • "Diagnosis is currently used": Selected by default. To deactivate the diagnosis, click to clear the selection.
      Note: Deactivating a diagnosis makes it unavailable for selection in encounters.
  3. Click Save. Repeat the steps for any additional new diagnosis codes. Once finished, log out and back into the Desktop Application (PM) to use the new diagnosis code(s).

Edit Diagnosis Code

  1. Find the diagnosis then double click to open. The Edit Diagnosis window opens.
  2. Make the necessary changes.
    Note: Only local settings are editable for Tebra subscribed diagnoses. For more customization, create a new diagnosis.
  3. Click Save. Log out and back into the Desktop Application (PM) to see the changes when using the diagnosis.

Deactivate Diagnosis Code

  1. Find the diagnosis then double click to open it. The Edit Diagnosis window opens.
  2. Click to clear the "Diagnosis is currently used" checkbox.
  3. Click Save. The diagnosis is deactivated. Log out and back into the Desktop Application (PM) to see the diagnosis is no longer available for selection in encounters.
