UB-04 Institutional Rejection

The Admission Date/Hour has been populated onto the encounter and needs to be removed.

It may be used only on inpatient claims (and some outpatient claims) as defined by the NUBC (National Uniform Billing Committee).

Resolution

Follow the instructions below to remove the Admission Date on the encounter:

  1. Click Encounters > Track Claim Status. The Find Claim window opens.
  2. Look for and double-click on the encounter that needs correcting. The Edit Claim window opens.
  3. Double-click on the Encounter number.  The Edit Encounter window opens.
  4. Select the Date field under the Admission section and click Delete on your keyboard.
  5. Verify the Date field now displays None.
  6. Select the Hour field under the Admission section and click Delete on your keyboard.
  7. Verify the Hour field is blank.
  8. Select the Discharge Hour field under the Admission section and click Delete on your keyboard.
  9. Verify the Discharge Hour field is blank.
  10. Click Save & Rebill.

Then, resubmit all affected claims.
