Add Vaccine

Document the patient's administered vaccines in the office, a previous administered vaccine, or document the reason why a vaccine was not administered. If necessary, order a vaccine to administer at a later date.

Access Immunizations

  1. Enter the first 2–3 letters of the patient's name in the top search box.
  2. Click to select the patient from the auto-populated results. The patient's chart opens.
  3. Click Immunizations in the left menu. The Immunizations page opens.
  4. Click + Vaccine to order, administer, add historical, or document not administered vaccines. The Vaccine page opens.

Order New Vaccine

If the vaccine is not available in the office, order the vaccine and document the order in the patient's chart. When the vaccine is available, administer the vaccine.

  1. Enter the Vaccine and select from the auto-populated results.
  2. Select the Type from the drop-down list.
    Note: The type populates based on the vaccine selected.
  3. The Ordered date populates with the current date and time. If necessary, select a different date or time.
  4. Select the Provider ordering the vaccine from the drop-down list.
  5. Optionally, enter applicable Comments for the vaccine.
  6. Click Save when finished.

Administer New Vaccine

Document a vaccine that was administered.

  1. Click to select "Administer New".
  2. Enter the Vaccine and select from the auto-populated results.
  3. Select the Type from the drop-down list.
    Note: The type populates based on the vaccine selected.
  4. The Brand is auto-populated based on the Type selected. If there are more than one brand available, additional brands can be selected from the drop-down menu.
    • The Mfr (manufacturer) is auto-populated based upon the Brand selected.
    • The Generic is auto-populated based on the Vaccine, Type, and Brand selected.
  5. The Ordered date populates with the current date and time. If necessary, select a different date or time.
  6. The Recorded date populates with the current date and time. If necessary, select a different date or time.
  7. Enter the Dose administered.
    • The Units and Route fields are auto-populated based on the vaccine selected.
  8. Select the location of the injection from the Site drop-down.
  9. If the vaccine is a multi-part series, indicate which Number of the Series is being administered.
  10. Enter the vaccine Lot Number as stamped on the label.
  11. Select the vaccine Expiration Date.
    Note: If the vaccine expiration date does not include a specific day, select the last day of the expiration month for documentation.
  12. Click to indicate whether consent for the administration was "Verbal" or "Written".
  13. Select who administered the vaccine from the Administered By drop-down.
  14. Select the Service Location where the vaccine was administered.
  15. Select the Provider who ordered the vaccine be administered from the drop-down.
  16. If applicable, select the date of the Vaccine Information Statement from the VIS Date drop-down.
  17. If applicable, select the Vaccine For Children financial class from the VFC financial class drop-down.
  18. If applicable, select the Vaccine Funding Source from the VFS Source drop-down.
  19. Optionally, enter applicable Comments for the vaccine.
  20. Click Save when finished.

Add Historical Vaccine

Document a vaccine that was previously administered.

  1. Click to select "Add historical".
  2. Enter the Vaccine and select from the auto-populated results.
  3. Select the Type from the drop-down list.
    Note: The type populates based on the vaccine selected.
  4. Select the Date the vaccine was administered.
  5. If available, enter the Number in Series.
  6. If available, enter the name of the provider who administered the vaccine.
  7. Select the Source of Information from the drop-down.
  8. Optionally, enter applicable Comments for the vaccine.
  9. Click Save when finished.

Add Not Administered Vaccine

Document the reason why the vaccine cannot be administered.

  1. Click to select "Not Administered".
  2. Enter the Vaccine and select from the auto-populated results.
  3. Select the Type from the drop-down list.
    Note: The type populates based on the vaccine selected.
  4. The Recorded date populates with the current date and time. If necessary, select a different date or time.
  5. Click to select the Reason for non-administration.
    • If "Other" is selected, enter the specific reason the patient declined the vaccine.
  6. Optionally, enter applicable Comments for the vaccine.
  7. Click Save when finished.
