Service Location : Facility point of origin and destination ambulance

Rejection Details

This rejection indicates an invalid zip code was sent for the ambulatory pick-up or drop-off address.


Verify the 9-digit zip codes submitted on the claim are valid. All addresses need to be verified using the USPS zip code lookup tool. 

Follow the steps below to verify the ambulance pick up and drop off addresses:

  1. Click Encounters > Track Claim Status. The Find Claim window opens.
  2. Look for and double click the appropriate claim to open. The Edit Claim window opens.
  3. Double click on the Encounter ID number.  The Edit Encounter window opens.
  4. Scroll to the Ambulance section and click the double arrows to expand.
  5. Use the USPS zip code lookup tool to verify the Pick Up Address and Drop Off Address and edit as necessary.
  6. Click Save & Rebill. Then, resubmit all affected claims when ready.
