Patient Demographics allow a practice to add information to a patient’s profile, edit the patient’s responsible party and emergency contacts, and view a patient’s appointments.
Access Demographics
- Enter the first 2–3 letters of the patient's name or date of birth (mm/dd/yyyy) in the top navigation bar search box.
- Click to select the patient from the auto-populated results. The patient's Facesheet page opens.
- Click Demographics. The Demographics page opens.

Navigate Demographics
- Click on the following tabs to edit:
- Profile: Enter patient information, such as driver's license number, marital status, and pronouns.
Note: The Tebra Patient ID is system generated when a patient chart is created.
- Additional Info: Enter additional information such as language, nationality, place of birth, preferred pharmacies, and primary provider.
- Contacts: Enter responsible party and emergency contact. By default, the Responsible Party is set to "Self" and the Emergency Contact is set to "Responsible Party."
- Click Upcoming Appointments or Past Appointments to view the patient's appointments.
- Click Patient Portal to Activate Patient Portal. This allows the patient to view their problems, allergies, medications, lab results, and vital signs.
