Navigate Patient Demographics

Patient Demographics allow a practice to add information to a patient’s profile, edit the patient’s responsible party and emergency contacts, and view a patient’s appointments.

Access Demographics

  1. Enter the first 2–3 letters of the patient's name or date of birth (mm/dd/yyyy) in the top navigation bar search box.
  2. Click to select the patient from the auto-populated results. The patient's Facesheet page opens.
  3. Click Demographics. The Demographics page opens.

Navigate Demographics

  1. Click on the following tabs to edit:
    • Profile: Enter patient information, such as driver's license number, marital status, and pronouns.
      Note: The Tebra Patient ID is system generated when a patient chart is created.
    • Additional Info: Enter additional information such as language, nationality, place of birth, preferred pharmacies, and primary provider.
    • Contacts: Enter responsible party and emergency contact. By default, the Responsible Party is set to "Self" and the Emergency Contact is set to "Responsible Party."
  2. Click Upcoming Appointments or Past Appointments to view the patient's appointments.
  3. Click Patient Portal to Activate Patient Portal. This allows the patient to view their problems, allergies, medications, lab results, and vital signs.
