Rejection Details

Tebra Validation Error

This rejection indicates the claim contains a Not Otherwise Classified (NOC) code that requires a detailed text description.

Claims billed without a description for a NOC code will reject at the clearinghouse; therefore Tebra has held the claim until the error is corrected. Once the error is corrected, resubmit the claims in order for them to be sent to the clearinghouse.

Note: Since claims rejected within Tebra are not forwarded to the clearinghouse, timely filing cannot be generated for the submission until the validation errors have been corrected and the claims are successfully submitted to the clearinghouse.

Follow the steps below to enter an NOC code description:

  1. Click Encounters > Track Claim Status. The Find Claim window opens.
  2. Look for and double click the appropriate claim to open. The Edit Claim window opens.
  3. Double click the Encounter ID number. The Edit Encounter window opens.
  4. Customize the Procedures section to add the Line Note and Ref. Code columns.
  5. Enter the information for the service line that needs correction.
    1. Ref. Code: Select ADD - Additional Information from the drop-down menu.
    2. Line Note: Type "NOC" in all caps followed by a space. After the space, enter the information (not to exceed 76 characters) as required by the payer.
  6. Repeat step 5 for all service lines that require a NOC code description.
  7. Click Save & Rebill.

Then, resubmit all affected claims when ready.
