Export Continuity of Care

One of the long term objectives and regulations for improving patient care coordination requires all healthcare providers to communicate with one another and securely share patient health information. Download the patient's Continuity of Care Document (CCD) as an XML file to share the patient's record.

Tip_Icon.png Tip: Send the Continuity of Care directly to the provider via Direct Messaging or eFax from the patient's signed clinical note. Certain CMS Incentive Program and some health IT regulations require the CCD to be sent via Direct Message.

The Continuity of Care file includes:

  • Basic Demographics
  • Problems
  • Social History (specifically smoking status)
  • Instructions
  • Allergies
  • Encounter Diagnosis
  • Care Plans
  • Clinical Notes
  • Functional/Cognitive Status
  • Procedures
  • Vital Signs
  • Medications
  • Medications Administered During Visit
  • Results
  • Immunizations

Access Export Continuity of Care

  1. Enter the first 2–3 letters of the patient's name in the top navigation search box.
  2. Click to select the patient from the auto-populated results. The patient's Facesheet page opens.
  3. Click the More drop-down arrow and select Export Continuity of Care. The Export Continuity of Care pop-up window opens.

Export Continuity of Care

  1. Click Export. The XML file is saved to the computer.
  2. The file is ready to be shared with applicable providers.
