Import Summary of Care

One of the long term objectives of the HITECH Act is to improve patient care coordination; this requires all healthcare providers to communicate with one another and share patient records. After a Summary of Care is received for the patient, import the XML file and merge the information into the patient chart.

The Summary of Care file is more comprehensive than the Continuity of Care Document (CCD)/Continuity of Care Record (CCR) file. The Summary of Care file includes:

  • Basic Demographics
  • Problems
  • Social History (specifically smoking status)
  • Instructions
  • Allergies
  • Encounter Diagnosis
  • Care Plans
  • Functional/Cognitive Status
  • Procedures
  • Vital Signs
  • Medications
  • Medications Administered During Visit
  • Results
  • Immunizations

Access Import Summary of Care

  1. Enter the first 2–3 letters of the patient's name in the top navigation search box.
  2. Click to select the patient from the auto-populated results. The patient's Facesheet page opens.
  3. Click the More drop-down arrow and select Import Summary of Care. The Import Summary of Care pop-up window opens.

Import Summary of Care

  1. Click Browse to locate and select a XML Summary of Care file.
  2. Click Upload. A confirmation message displays and the file is uploaded to the patient's Documents page.

Merge Summary of Care

  1. Click Documents. The Documents page opens to the Documents tab.
  2. Click Merge next to the imported Summary of Care. The Merge Information pop-up window opens.
    • If this is the first time the user is merging a Summary of Care, the Merge Wizard opens. Review the Merging Patient Information Guide to proceed.

  1. Compare the patient information received from the other provider to the documented patient information in Tebra.
    • The first column contains patient information from the imported Summary of Care.
    • The Clinical column contains information currently documented in the patient's chart in Tebra.
    • The Preview column displays a preview of items that will be merged and/or removed.
  2. To merge an item into the patient chart, click to select the item. The green merge icon displays in the Preview column.
    Tip_Icon.png Tip: To import all items in the section, click to select the section header.
  3. To exclude an item, click to clear the item.
  4. To deactivate an item currently documented in Tebra, click to clear the item. The red remove icon displays in the Preview column.
  5. Click Save when finished. The reconciliation successfully saved message displays and all merged items are now available in the patient's chart.
