Assess a patient's mental health progress by reviewing completed screening tools (PHQ-9, GAD-7, PCL-5, and AUDIT) via Patient Intake. Providers can track the patient's historical scores and compare them with the most recent values. This allows the provider to quickly identify any problems and make necessary adjustments to the care plan.

Providers also has the flexibility of creating a flowsheet with the type of data and layout that makes sense for their practice.

Note: Scores from screening tools completed within a note will not populate in this flowsheet.

Available for Engage subscribers and accounts with access to Patient Intake by December 5, 2025.

Access Mental Health Flowsheet

Enter the first 2–3 letters of the patient's name in the top search box.
Click to select the patient from the auto-populated results. The patient's Facesheet page opens.
Click Flowsheets in the left menu. The Flowsheet page displays.
Click Flowsheet drop-down arrow > Mental Health.
By default, the date range is set to 1 year.
- To change the date range, select a different pre-set date range or set a custom date range. Then, click Update to refresh the data.

Review Mental Health Flowsheet

Data in the flowsheet populates from the completed screening tools via Patient Intake.

If the screening tool was completed multiple times in the same day, the results will stack with the most recent result on top and oldest on the bottom.

Available screening tool data:
- Patient Health Questionnaire-9 (PHQ-9)
- General Anxiety Disorder-7 (GAD-7)
- Post Traumatic Stress Disorder Checklist for DSM-5 (PCL-5)
- Alcohol Use Disorders Identification Test (AUDIT)
The dates correspond with the completed patient intake date.
