Send Intake Forms from Confirm Appointment Request

Patients can request an appointment through the practice online scheduler or the provider profile page. Confirm the requested appointment to schedule the patient on the provider's calendar. The front office can send an invitation to complete patient intake forms along with the appointment confirmation email.
Available for Engage subscribers and accounts with access to Patient Intake.
Access Tentative Appointments
- On the Dashboard, click Tentative Appointments. The Appointment Statues page opens.
- Click Review. The Confirm Appointment Request page opens.

Confirm Appointment
- If applicable, select a Visit Reason based on the Note from Patient. The Duration updates to the duration set for the selected visit reason.
Tip: Enable visit reasons for patients to select on the provider's Tebra Care Connect profile or the Practice Online Scheduler.
- Click the Select additional forms drop-down arrow to select which patient intake forms to send. Available forms:
- Select the method of delivery:
- By default, the option to send the patient intake form by SMS is selected. If necessary, click to clear.
- To send the patient intake form via email, click to select "Email".
- If applicable, click the Edit icon to update the patient's email and/or mobile phone number.
- Click Confirm Appointment when finished. The patient receives an appointment confirmation email and an email and/or text invitation to complete the patient intake form online.
