Percentage of patients 3-17 years of age who had an outpatient visit with a Primary Care Physician (PCP) or Obstetrician/Gynecologist (OB/GYN) and who had evidence of the following during the measurement period. Three rates are reported.

Percentage of patients with height, weight and body mass index (BMI) percentile documentation
Percentage of patients with counseling for nutrition
Percentage of patients with counseling for physical activity

Denominator:	Patients 3-17 years of age by the end of the measurement period with at least one outpatient visit with a primary care physician (PCP) or an obstetrician/gynecologist (OB/GYN) during the measurement period. Patient’s age is calculated based on the patient’s Date of Birth, which was entered when you created the patient’s account. Create a note detailing the patient’s office visit.
Denominator Exclusions:	If patient is pregnant during the measurement period: "Pregnancy" is documented under Problems with a Start Date. These patients will be excluded from the denominator. If patient was in Hospice during the measurement period: Hospice is documented under Hospice in History. These patients will be excluded from the denominator.
Numerator 1:	Patients who had height, weight, and body mass index (BMI) percentile recorded during the measurement period. BMI is calculated based on Height and Weight entered in Vitals.
Numerator 2:	Patients who had counseling for nutrition during the measurement period. "Counseling for Nutrition" is documented on the Care Checklist.
Numerator 3:	Patients who had counseling for physical activity during a visit that occurs during the measurement period. "Counseling for Physical Activity" is documented on the Care Checklist.
Office Visit Documentation:	Enter Procedure Codes including E/M Codes, HCPCS, or CPT Codes as applicable on the Charge Capture Complete and sign the note Once all steps have been completed, the measure will be added to the Quality Measures Report.
