Clinical staff can review the patient's notes, psychotherapy notes, and treatment plans based on their permissions.
Permissions to the Notes page may vary based on the practice and provider's subscription, user's roles, or chart view customizations.
Access Notes
- Enter the first 2–3 letters of the patient's name or date of birth (mm/dd/yyyy) in the top navigation bar search box.
- Click to select the patient from the auto-populated results. The patient's Facesheet page opens.
- Click Notes. The Notes page opens.

- Treatment Plans: The section only displays when a treatment plan is created for the patient.
- Display: Filter by Active or Marked as Error treatment plans.
- Treatment Plan Details: Displays the status, the assigned provider plan, and the start date with the date the treatment plan was last updated. Available status:
- Action Item: Click Open to view the treatment plan.
- Notes: Displays all notes (e.g., clinical notes, memos) for the patient.
Note: A lock icon displays when another user is accessing the note. If necessary, advise the user to unlock the note.
- Filters: Select filters to view specific notes.
- Sort by: Sort by Date of Service or Last Update.
- User: Display notes for All users or by a specific user.
- Display: Display Active, Marked as Error, or CCD Imports notes.
- Note Type: Display All note types or by a specific note type.
- Note Details: Displays the note type, the chief compliant, the status, the assigned provider, the date of service with the last edited date, and if the note was AI-Assisted. Available status:
